Everyone is entitled to an assessment of their social care and support needs.
We begin this process when you first make contact with us, via our contact service for social care.
Depending on your needs we can give you advice and signpost you to organisations who may be able to help, or if your needs are greater, we can arrange for a Community Care Assessment to take place.
When you receive a community care assessment we will
- Explore your individual social care needs
- Involve you, and your carer if you have one, in the assessment process
- Identify risks to your independence, health and safety, daily routines and community involvement
- Take into account any additional needs you may have due to your personal circumstances such as language differences, cultural or religious background, specific disability or illness, responsibility for dependants, age, gender or sexuality where appropriate
- Include a financial assessment
- Treat you fairly, whatever your circumstances, in line with current policies and standards
- Identify your strengths and abilities
- State the outcome of the assessment
