Hospice care

Service Outline

Hospices provide care specifically for terminally ill people and give support to their carers both in the hospice and at home.

How does it work?

Manchester's care services in this area are multi-agency teams. Care managers working for the Council work closely with the NHS, and Macmillan Cancer nurses.

Referrals are made by NHS ward staff and the council's care managers, as part of a community care assessment may refer you to the Outreach nurse where appropriate.

Care managers, Macmillan nurses and GPs all have up to date lists of hospices which might be suitable, and they can discuss this with you if they think it's appropriate for you to use a hospice.
