If you are going into hospital you may need help when you leave to continue to live in your home.

Manchester City Council's Directorate for Adults, Health and Wellbeing can arrange an assessment of your support needs when you leave hospital. Depending on the outcome we can offer different levels of support. Read more about by following the link below.
