Summary
A Personal Licence allows an individual to be a Designated Premises Supervisor (DPS) at a licensed premise that sells or supplies alcohol.

Validity / Renewals
A Personal Licence is valid for 10 years from the date it is granted and is portable throughout England and Wales.
Application should be made to the Local Authority in which the applicant lives, and not to the Authority in which the licensed premise is located.

Relevant legislation
Applicants must not be under 18 years of age, and not had a Personal Licence forfeited within the last five years.
You must hold an accredited licensing qualification and must declare any convictions for any relevant or foreign offence.

Attachments to be submitted with your Application
a) a criminal conviction certificate or criminal record certificate or the results of a subject access search of the police national computer by the National Intelligence Service;
b) a disclosure of convictions and declaration form;
c) two photographs of yourself, one which is endorsed as a true likeness of you by a solicitor or notary, a person of standing in the community, or any individual with a professional qualification; and
d) the original or a certified copy of your Level 2 qualification for Personal Licence Holders.

Declaration
Upon completion of your application you will be required to confirm that you:
a) have read fully and understood the contents of this form and any supporting information and agree to the terms and/or conditions set out within;
b) understand that by submitting this application, you consent to Manchester City Council passing on the data within this application and its supporting documents to any authority or person that will assist its determination of this application, or as required by law;
c) declare that the information provided is true to the best of your knowledge and belief;
d) understand that if any false information is provided you may be guilty of an offence and liable to prosecution;
e) understand that if the application is authorised you must inform the authority about any changes to circumstances that mean you no longer meet the conditions for the authorisation;
f) understand where a statutory declaration has been provided it may be forwarded to Greater Manchester Police;

Processing Timescales
If you have any relevant convictions the Licensing Unit will notify the police of your application. The police then have 14 days to object to the application, if they choose to do so.
If the police do not object to the application and the applicant fulfils the criteria, the licence will be granted.
