If your organisation has received funding from the Council, your grant offer or funding agreement will have explained the monitoring requirements. If you need a hard copy of the monitoring form to be posted to you please email the following information to: voluntary.sector.grants@manchester.gov.uk

The name of your group/organisation

The address of the group/organisation or the address to send the form
Forms when completed can be emailed back to voluntary.sector.grants@manchester.gov.uk
If you do not have a digital signature for the management committee member signing on behalf of your group you must also send in a hard copy of the signature page by post to the Area Co-ordination and Third Sector Team.
