Your ‘pathway plan’

When you reach 15 ¾ your social worker will work with you to create what we call a ‘pathway plan’. This is a plan of how everyone will work with you to give you any help you want to become fully independent.

Together you will complete an assessment of all the skills you have, and anything you might need to learn to live independently. They'll use that information to develop your pathway plan with you.

When it’s finished your plan will contain details of the support (including financial support) you can expect and will replace your care plan when you turn 16.

Remember this is your plan — for, and about you. You know best what direction you want your life to take, so make sure that you’re involved and have your say.
