What happens next?
We will send you a form to fill in - please give as much detail as possible, including names and addresses of people or agencies that give you support.
When we get your form we will decide whether we need to visit you at home to check your circumstances. If we have enough information on the form we may make a decision without seeing you - but we may contact other agencies to clarify your condition. If you live outside Manchester (see map) we will not visit you at home.
