- a family member in your household is terminally ill.
You will qualify if you have a completed SR1 form (Special Rules 1 – confirming a terminal illness). Find out more about the SR1 form on the Marie Curie website.
Any medical professional who has completed the SR1 can apply on behalf of the person who is terminally ill. This includes GPs, healthcare workers or support staff.
To apply:
The medical professional or support worker must:
- email benefit.enquiries@manchester.gov.uk. Attach a copy of both sides of the completed SR1 form.
- Include your council tax account reference number (or a household member’s) if you know it. You can find this on any letter about your council tax. Or;
- if you can't find your council tax reference number, include your home address.
