£29 per licence, includes document appointment, licence and badge.

We aim to process your application within 10 working days of receiving the application and all supporting documentation.

You will need the following to process a change of name:
- DBS disclosure (formerly known as CRB.) Use the online form above to request;
- DVLA mandate for release of information, this is within the application form above; and
- Deed poll certificate or other legal document i.e. marriage certificate, confirming your change of name, photocopies are not accepted.
