Change of name - hackney carriage/private hire

If you change your name you must inform us immediately and your records will be updated.

£29 per licence, includes document appointment, licence and badge.

To change your name there are two forms to process:

1) The change of name notification and DVLA mandate; and

2) DBS disclosure application; follow the new online process.

You will need the following to process a change of name:

• DBS disclosure (formerly known as CRB.) Use the online form above to request;
• DVLA mandate for release of information, this is within the application form above; and
• Deed poll certificate or other legal document i.e. marriage certificate, confirming your change of name, photocopies are not accepted.

We aim to process your application within 10 working days of receiving the application and all supporting documentation.
