Book a birth registration appointment

Use this page if your baby was born in Manchester.

Complete the steps below before booking your appointment.

Step 1: Have the birth details ready

You will need:

  • your baby's date of birth
  • your baby's place of birth
  • your baby's first name or names and surname
  • the exact spelling of all names
  • the mother's full name
  • the mother's date and place of birth
  • the mother's occupation
  • the mother's maiden name, if applicable
  • the date of marriage or civil partnership, if applicable
  • the same details for the father or second parent, where their details will be recorded.

Step 2: Bring proof of identity

From 9 November 2026, anyone registering a birth must bring proof of identity to their appointment.

If the parents are married or in a civil partnership, only one parent needs to provide proof of identity.

If the parents are not married or in a civil partnership, both parents must provide proof of identity if both attend the appointment.

If you do not bring the correct proof of identity, we will not be able to complete the registration. You will need to make another appointment.

What you must bring:

See the lists of items on this page. You must bring either:

  • one document from List 1; or
  • two documents from List 2.

Photographs and photocopies are not accepted.

The name on your identity documents must match the name you want recorded in the birth registration.

If you use more than one name, you must provide accepted proof for all names you currently use.

Step 3: Bring your baby's NHS number

Bring either:

  • your baby's Personal Child Health Record (red book); or
  • another document showing your baby's NHS number.

The hospital will send us a record of the birth.

Step 4: Check the details carefully

During your appointment, we will record the information you provide.

You will be asked to:

  • check the information we have recorded
  • make a verbal declaration that the information you provided and the information we recorded is accurate.

Check names, dates and spellings carefully.

Mistakes can be corrected later, but you may have to pay a significant fee.

Step 5: Buy any certificates you need

Birth certificates cost £12.50 each.
