Support for Young People with Complex Needs and Disabilities

Between the ages of 16 and 18, we begin the transition process from Children’s to Adult Services. Here’s how we can support you:

Transition Process

  • Transitional Assessment: You can request your ‘Transitional Assessment,’ which will identify the support you need for a happy, safe, healthy, and successful future.
  • Independent Advocacy: If you would like support, we can help you access independent advocacy services.

Financial Management

  • Appointee Confirmation: If you already have an appointee managing your finances, please confirm three months before your 18th birthday whether they will continue to do so once you turn 18. If not, you will need to identify someone else to act on your behalf. You can discuss this with your Social Worker.
