Apply for chaperone approval

Only complete this application for approval if you are a permanent resident within Manchester. You must live in and pay council tax to Manchester City Council, if you are not sure check which council.

Important information you need to know before you complete this form:

To be registered as a chaperone with us you must have a Disclosure Baring Service (DBS) (formerly known as CRB) – this must either be issued by Manchester City Council (there is a charge for this) or you already subscribe to the DBS update service, you will be asked for this information within the process.

If you are a volunteer chaperone it will be beneficial if you subscribe to the DBS update service, when you make your application. We recommend you do this as it will keep your DBS valid (this means you don’t need to complete another application) as this service is free for volunteers – see DBS update service for more information. 

Before you use this form - the following items are required to be submitted with your application:

  • Passport style/size photograph (taken within last 3 months); 
  • Photo ID (driving licence or passport);
  • 2 references (written within the last 3 months), one in a professional capacity or knowledge of relevant experience (paid or unpaid work) within the last 2 years and one from a person who has known you for at least 2 years who must not be a relative or a person living at the same address as you. The references must be completed using the Chaperone reference template and uploaded within this process - we will contact all referees to confirm the information provided; and
  • DBS certificate - if you subscribe to the DBS update service.

Your name and contact details will be kept on a chaperone register. These may be passed onto other local authorities, TV companies, theatres and entertainment agencies as and when requested.
