Your Pathway Plan

You’ll work with your Personal Adviser to write your Pathway Plan. The plan will include information to help you transition from care to living independently.

  • We will start developing the Pathway Plan while you are still in care – within 3 months of your 16th birthday.
  • It sets out how we will help you to achieve the things you want to do.
  • You will be fully involved in preparing and reviewing your Pathway Plan.
  • We must review your Pathway Plan with you at least every six months until you reach 21.

In your Pathway Plan we will include information on:

  • education
  • training
  • employment
  • well-being
  • your home
  • family and social relationships
  • money
  • health
  • your ‘self-care’ skills
  • identity

Make sure you talk to your PA about any needs or plans you have so they’re included in your Pathway Plan.
